We are a multi-specialty group that had been using iO Practiceware for almost three years. I was initially apprehensive, mainly because of stories I had heard of much decreased productivity after adopting EMRs. I have been thrilled to find that the opposite is true. In fact, by the third day after implementation, I was able to see a full load of patients with only a thirty minute lag. However, I do believe this success is entirely system-dependent. I ran into a physician that had used iO Practiceware in our office and then moved out of state to a practice that used another EMR (one which claims to have the highest number of installations in ophthalmology). He was extremely frustrated because after six months, it was still taking him twice as long to enter patient data. The drop down menus he was using in his EMR were very cumbersome. iO Practiceware, on the other hand, uses a touchscreen system which is lightning quick and simple to use. At this point, I feel it would be most instructive to list the attributes of iO Practiceware that make it so efficient to use: 1. The system is well thought-out and organized. There is an alert button which allows important "notes to self" to be placed on a separate screen. For example, I use a treat and extend protocol on exudative macular degeneration patients. I have a note in this section indicating the current treatment interval for a given patient. This alert screen can be accessed with a push of a button from any other screen. This precludes the need to flip back to previous visits to find a note indicating a patient's treatment interval. 2. There is a highlights screen which is very well organized. It lists the previous procedures and surgeries done on a patient. The previous retinal drawing is also shown. From this screen, a chronological list of previous visions and pressures can be accessed by touching the current vision or pressure on that screen. The font size used on this screen is very large so that information can be seen at a glance from even 5 feet away. 3. A separate interpretation and report for testing such as OCT or FA can be accessed at any time. Also op notes for lasers can be accessed at any time. 4. Referral letters are easy to generate and an updated list of physician phone numbers are in the system. The letters are sent by a simple push of a button. 5. The system is easily customizable. A computer savvy person can add treatment templates in a matter of minutes without having to hire an expensive programmer. 6. The monthly maintenance agreement cost was very reasonable when compared to other systems we looked at. 7. Service is very personalized in that we are usually able to work with the same person on an ongoing basis. 8. Information is stored on an on-site server rather than in the cloud. This has many advantages in terms of speed of information access. There is virtually no lag when images are retrieved. 9. Documentation of patient information is extremely thorough. All bases are covered. In fact, a practice consultant that does practice self-audits remarked that this was the best system she had encountered so far in terms of documentation. In summary we have been very happy with iO Practiceware. It has increased my efficiency by 10 to 15 percent over paper charts. Also, documentation is a hundred percent better than patient charts.

1st of all we want with this software due tot capability to integrate directly to what seemed like a quality EHR system. The billing features would only be further enhanced once we could implement EHR down the road, however we purchased this program to use only for billing initially. We were sold on the "lcustomer service calls answered live 98% of the time". The reality was about the exact opposite. We can call numerous times in a day, either going straight to voicemail, or holding for 20 minutes then a voicemail and never recieve a call back & rarely within the same day, so we just keep calling. Further, our issues never seem to be a high priority to them, even when they feel URGENT to us. The week we implemented our contract, we were assigned a trainer, and received emails stating she was out of the office for the next 3 weeks with limited phone and email access. Nice start, huh!? & they had new phone sysytem so we also couldn't get customer service to answer the phone. We called our sales rep, and he was out to. We called and emailed other trainers, similar replies, out for weeks. Bottom line, M-plus took on to much, to soon and was not & &/ still not prepared, staffed & capable of servicing and training new customers. We have been with them now for going on 10 months with severe program issues due to lack of training and information. We schedule a training and they want to reschedule. They want to do everything on a recorded webinar when we need direct training & specific directions. There are significant shortcomings we are still coming across. Granted they have customers of 16 years who are happy but right now they are not capable of helping new customers. They are not personable & don't make their customers feel like their business really matters. In attending a recent users group meeting, there was nothing of special note to make us feel warm & fuzzy about being a part of their program. After all the problems we have had, even the CEO said "let me know if you need anything" but didn't really respond much when I indicated they completely missed many sales opportunities by not taking care of us from day one! (As it had been our intention to go EHR by year end)

No up front cost, you pay monthly for the program. You can create about any report, once you learn to think like their program. Billing, EHR & Optical & batch eligibility. You don't pay extra for customer support

You can't get live customer support as they promise. You do ALL the set up on your own (something other/older customers told me they didn't have to do, but apparently now new customers, or at least we did) The used a excel of patients & insurances & uploaded the bare basics. All the set up of insurances and clearing houses even the specifics of many of the loop and segments have fallen upon us to know & work through... again a very difficult, painful & unncessary process. Even when we are told how/what to enter, it has been inconsistent & had to be changed multiple times & as of this week, a program change now renders it ALL to be reprogrammed yet again... not by them, but now I have to go back to every single insurance & make changes to avoid denials...

#### Pros

The system does what I need it to do when I need it to do it. It's fast, efficient, precise, customizable and the tech support has been just as responsive. Unlike other EHR/EMR systems I researched, MVE worked with me on cost and let me add modules as I realized I needed them. The big companies have a set price for a set system (all or nothing out the door). I have now purchased almost every module that MVE offers and because I was allowed to experience each module separately I gained the skills and confidence to drive this system efficiently. I am just now beginning to appreciate the agility and resilience of the MVE programs. The onboarding team (your trainers) are the best in the business, as far as my staff and I are concerned. We call, they answer. I'm not giving up our trainer's name because he is the best of the best. He actually can work this system to its max and makes learning fun and simple. He remotes into our system and not only demonstrates the modules but makes us repeat the scenarios while he is logged in. He let's me show him all the damage I've done with the "customizer" button. And the best part - after hitting all the wrong buttons, deleting essential programs, and creating chaos in the name of "just because I can", the MVE team steps in and restores order and functionality; usually within a couple of hours depending on what time we call/email. MVE has the best video tutorials out there in addition to the live help.

"Gigo" garbage in - garbage out. If you don't take time and really learn how to manipulate MVE you will be dissatisfied. Thinking that you are going to buy this and it will magically read your mind will lead to big unhappiness. Like any AI (it will give back what you put in).

First of all, I must preface my comments by the fact that I am a solo practitioner, Retina practice only, seeing roughly 30 to 40 patient encounters a day. In an attempt to be objective about the various offer, I separate software itself from connectivity issues, as well as the overall frustrations with the need for EMR, PQRS, and MU governmental regulations, which already cast negative cloud over the entire EMR implementation process. What enticed me about this product was the touchscreen interface, in addition to endorsement by a large retina group we know. We also decided to go for a cloud-based EMR rather than in-office servers. Most importantly, we prepared for implementation for three months prior to the same, per the recommendations of iO. I believe that if your implementation is rushed with any EMR system, there will be issues that will be difficult to resolve instantaneously. Having said this, from the time of implementation, it took us only several weeks before I was up to my regular patient load without a scribe! After synchronizing our billing department and its processes with iO, billing has been more efficient than pre-EMR; payments have been faster as well. Of note, we implemented in September 2014 before the last-minute rush; we had started preparing from July 2014 for implementation. Although we started with templates from another practice, which were quite comprehensive, I've been able to tailor the templates myself over the last several months, which makes patient assessment/plan summaries more efficient.

#### Cons

There are a good number of steps needed from seeing the patients to check out. If I look at this objectively, however, most of these have to do with PQRS/PQR I and MU requirements. I'm able to make this more efficient by rearranging the button/templates, which can be done quite easily. Initially, support had been sparse. Recently, I am able to speak with an individual, as is my office staff, in a reasonably efficient manner. There have been issues with dropped connectivity on occasion. We are troubleshooting the same with iO; however, we are cloud-based, and I am here in the Midwest. This does not appear to be as much of issue with the software, as it is with cloud-based connectivity; nevertheless, I would choose cloud-based over server-based in an office any time, as updates are done overnight. The company is constantly updating their software. The company does respond to requests regarding software updates/modifications, and the speed of this is improving.

#### Pros

At the optical where I work, we recently upgraded to a new billing/clinic software called MVE. I think this program makes the program we used to use look like the kiddie table of optometric computer software. It's easy to use, making medical billing 100x easier for staff. The exam script looks professional, and we can even add our logo to anything we print out! It's amazing. I love this software. It makes our jobs so much easier!

I do wish we had more education available about updates when they happen. Any software is going to have a few bugs but overall, I cannot complain much.

Where do I start...terribly implementation and support. No customization. It is not a "cloud based" software -- it is a windows program and only works with windows devices - not mac, iPhone or iPad. Super slow. Seriously looks like a software program from 1980's.

We have been using Management Plus since 2012 and have been extremely satisfied with our decision to purchase their complete system. Pros: User Friendly, budget conscious, complete PM, Optical & EHR integrated software, FANTASTIC customer service (located in the USA) and totally customizable to your practice. You don't have to wait until all customers want to change or add something to the EHR template. You can do it anytime. Not just fixed reports...You can customize your own reports which give you lots of flexibility when owners/doctors request specialized reports. We currently use IRIS Registry and have received all incentives through Meaningful Use Stage 2. Management Plus has integrated successfully and we do not have to report manually on CQMs or PQRS anymore. Cons: None come to mind

We have been using Management Plus for over 15 years and it just keeps getting better. The ability to customize the EHRs has been a great benefit for us. The support is top notch. I would recommend anyone jumping into the EHR pool to take a look at Management Plus.

The program itself is intuitive and user-friendly. The coding engine is extremely powerful. As a Neuro-ophthalmologist, I need a simple and quick way to document an arduous motility and alignment exam, and can also be easily accessed from prior visits and imported. Integrity EMR provides that ease of documentation in an intuitive interface. When a note is printed or a letter is generated for a referring physician, the printout is organized beautifully and reads like an Ophthalmology note is intended to be read. Integriview contains all imaging (HVF, OCT, FA, photos, etc) in a single location and is available with a single click of a button. In summary, Integrity EMR is the best EMR I have used for Ophthalmology.

Perhaps not an inherent weakness of the product itself, but when our servers are slow or the internet is down, the luxury and efficiencies of this EMR are lost. There is not a way to work "offline" and I must resort to using the ancient form of documentation called pen and paper.

I can't stress enough how poor the customer service is. I would not recommend this system to anyone in Ophthalmology based on our experiences with customer service.

The customer service has gone from bad to worse. The hold times when calling in are ridiculous only to be transferred to a voice mail. Also, if you email customer service support and are lucky enough to get a response back, it's usually a question about your question which only prolongs the issue you are having.

Our practice is a two-location, five-physician operation with a strong focus on refractive surgery. Our combined patient volume is 150-200 visits daily. While we were ready for the patient care benefits that we knew we could gain through EHR much sooner - we did not make our final selection until 2010. Our physicians selected Medflow as this was a platform designed specifically to meet the need of ophthalmic care. We paid specific attention to the user friendliness as well as customization (or lack of over-customization that can really become the Achilles heel of any EHR) of the system, thinking ahead and hoping not to have to require professional IT support at every upgrade to make the system update across all templates. Medflow fits those characteristics. We instantly took to the wonderful, single screen exam as well as refractive module that is utilized the most by our refractive surgeons for pre-, intra- and post-operative care. One of the main deciding factors was imaging integration - we love having DICOM in place that allows for error-free application of digital images - such as OCTs, Visual Fields, Corneal Topographies - directly into the EHR. Many other EHR systems require third party devices to accomplish the same thing. Our doctors and technicians enjoy using Medflow in daily patient care, and electronic charting has become the second nature for them. It is important to mention that Medflow integrated seamlessly with our Allscripts PM system, as well as patient portal via Vision Connect (that bridges both EPM and EHR). Highly recommend considering Medflow for any practice of ophthalmology!

I walked into a practice that already had iO Practiceware implemented, having come from a practice that was not using EHR. Seeing iO in use while applying for this job was one of the main reasons I joined my current practice. The ease of use and data input make it fast and efficient. While one patient is being worked up, I can preview their file and see who else is waiting to be seen. It really helps me keep track of my day. Their tech support team was very responsive. I recently wanted to implement a button that generated an H&P for all of my upcoming surgeries. One of their technicians has personally kept up with my request and seen it through. Needless to say, I can easily prepare my pre-op notes with a push of a button at this point. Another benefit compared to paper charts is one that we would rarely have had the benefit of at my old practice. If a call comes in at night about a patient, I can pull up any record from home with no trouble and find out all the details I need. Technicians aren't running around the office constantly looking for someone's misplaced chart. If a chart goes to a satellite clinic, I don't have to wait an hour for someone to drive it back to our main clinic. Needless to say, there are a lot of naysayers about EHRs, but with iO, I find that the interface and data availability is extremely user friendly and easy to work with. Admittedly, this review comes from someone that is fresh out of fellowship, but if you are looking to pick a good EHR to work with, I found learning the process extremely easy with iO.

The EMR has all the information needed for complete exams as well as specialty exams. Interface is busy but once you get used to it, it is very easy to navigate.

Awful customer service and support. The last update that was issued on December 8th had an unknown interface change with our GE billing software. On January 5th, we still have not been able to submit claims due to MedFlow's error. GE never requested any changes to the billing interface. MedFlow seems nonchalant about this problem, a problem that has prevented us from submitting claims for over 3 weeks now!

Our practice has 14 locations, a LASIK Suite, and a surgery center which is AAAHC accredited. We have a high volume of cataract practice and see approximately 350 patients a day in our rural clinical setting. We have been using Medflow EHR going on four years now. We were the first Ophthalmology practice in the country to meet meaningful use. I have found Medflow to be reactive and proactive to our needs, and the full screen exam very user friendly. We are on the newest version of Medflow. Implementation was a seamless event in our practice and was basically a celebration of all the hard work we put into it on the front end when we were getting the product ready for use. After a full week, we were back to seeing a full patient load. Many of the providers took themselves live prior to our actual go live date. They did that because it was easy for them to do so. We also use Medflow in the surgery center which has been an extremely nice tool for the staff. I have and would recommend Medflow of colleagues. I know that Medflow currently has the most providers who attested for meaningful use. That statement speaks volumes of what they have accomplished. We'd never go back to paper.

#### Pros

I love the cloud based platform of this product, especially since we have more than one site in which we see patients. We always have access to the patient's data, including when I'm on call. It also eliminates the need for EMR server maintenance.

#### Cons

I always look forward to the new features and updates. I'd like to see this more often but I know there are the meaningful use requirements that consume much of the development time.

Not much positive to say. Cloud based, but some reliability issues.

#### Cons

The Flexibility and Interface is very limited. The is no way to do cataract or refractive surgery planning. These capabilities were promised, but still not have been implemented- missing promised dates multiple times. The medication management/prescribing software is very bad. They acknowledge this but are locked into a third party system (Dr. First) and cannot change it. There are many run- time errors that occur. Image management is limited.

#### Pros

Management Plus is a fully-customizable EHR system. Most companies do not let the individual office make changes to their EHR. They have one EHR system for all users. Management Plus allows their users to customize the EHR to fit the users' individual offices. Their customer service is excellent and located in the United States. They also have Practice Management, Electronic Health Records, Electronic Prescribing, Patient Portal, and an Optical Module. You don't have to go to other software companies to purchase these items.

#### Cons

It would be great if they had a manual for using their product. It's not that I dislike that they don't have one, but it would be nice to be able to see what all the product will do for you as you have time to look through a manual.

The clinical aspect of the ophthalmology-specific software was good to very good, and the product also incorporates patient scheduling and some billing functionality.

#### Cons

The company has not invested into the much-needed technological achieve updates to keep it functional with software updates. Every time they perform an update, whether it's designed to address Meaningful Use or fix a known problem, the software's performance suffers in the most negative and impacting ways. The company has not hired enough support personnel nor have they trained their staff to focus on customer service. We've experienced long hold periods and a frequent lack of followup on open tickets. They point fingers at hardware and other support vendor companies.

Our office has been using IO since 2005. We first implemented the practice management system and then followed with the EMR in 2006. Our office has been totally paperless since. I wouldn't think of ever going back or using a different system. The system has simplified so many aspects of the way our practice operates. The are no more files to pull or lose. The system automatically bills for services as they are charted, so there are no lost charges. The system automatically completes the chart so that it is audit proof. It automatically generates letters to referring doctors based on the information charted. The system was designed by ophthalmologists so that charting is very easy. In my experience the time to produce a completed note is less than with paper charting. IO is very helpful in setting up the office and training the staff. We have been receiving our EHR incentive bonus, ERx and PQRI bonuses. IO helped simplify the process.
