### Four years in, the software was never the constraint

Four years on AdvancedMD, currently running about 70,000 appointments a month across a nationwide behavioral health group of roughly 200 people. My role there is Chief Digital Officer, so I own the technology and the operations and the P&L consequences of both. I've worked inside a number of other practices on this system over the years, some larger than ours, so this isn't a one-instance opinion. Most reviews are written in the first ninety days, when implementation is fresh, everything is hard, and you don't really know yet what you're looking at. Four years in, the picture is different. What determined our results was not the software choice; it was what got built on top of it. Same login, same screens, and our average cash conversion went from 76 days to under 10 across more than 200 payers, with clean claims above 95%. Every system hits a wall where the report you need doesn't exist and automation stops at whatever the vendor built. That wall isn't here. Two findings from practices I've worked with: $188,000 in visits sitting unsigned, every one an invoice that could not be billed, and $40,000 in late cancellation and no-show fees the practice was owed and never charged. That $40,000 is pure cash; no claim, no payer, no waiting, and with a card on file it collects immediately. Neither showed up in a standard report. Both were sitting in the system the whole time. If you're evaluating right now: demos are built around screens, because screens fit in an hour. The part of this system that matters most isn't a screen, so it never comes up. You can sit through four demos and leave with an accurate read on the interface and no idea what the system can actually do.

#### Pros

Real-time access to your own data, which is the whole ballgame at scale. Automation goes well past what ships in the box, so adding providers doesn't mean adding headcount. Scheduling holds up across locations and providers at volume, and the notes and forms are customizable enough that we built them around our workflow instead of the other way around. Single sign-on and SCIM provisioning are available, so staff log in with the same Microsoft or Google account they use for everything else and accounts get created and shut off automatically as people join and leave. That is a real security and administrative win once you're past a handful of users, and it's rarely on anyone's evaluation list. Handles complex payer mixes without complaint. We worked our EDI agreements payer by payer and eliminated 98% of paper billing. Four years, heavy growth, and it kept up.

#### Cons

Steep at first, and it prices above the small-practice systems, which is the complaint you'll see most. Nobody making that argument has run the return. Software is a rounding error against payroll, and if the system absorbs manual work and shortens the time from visit to deposit, the difference disappears inside the first quarter. Support is the part with the most room to improve; complex issues can take a few passes to land with someone who knows the depths of the system. Nobody's perfect there and I've seen worse. The bigger issue isn't the vendor's: most practices configure this once at go-live and never again, then blame the software for what the default settings do. This system rewards investment in setup more than any other I've used. Knowing how to point it at practice cash flow and then scale it to enterprise throughput and volume is the part almost nobody is looking at, and it's the part that decides what you get out of it. If you plan to stay exactly the size you are and never change how you run, you'll never touch most of what you're paying for.

I had an a great experience with AdvancedMD when I was with the company that used it. It was a great improvement from the last EHR we had used. AdvancedMD worked a lot smoother then our past EHR and there was less bugs which helped with productivity.

I appreciated that this EHR was a lot more efficient and customizable than the last EHR we were using. It made scheduling patient’s fairly easy and the patient portal was nice so that communication between patients and practitioners was integrated into the system. Customer support was very fast and efficient which was nice because when issued this system it was new for the whole company. The use of AI integrated into the system was helpful also. It was also very easy to learn and customize to our needs. Patient Management was also a lot smoother with this system then the last system we had used. It was nice to have zoom integrated into the system for virtual visits.

We did a lot of group scheduling which sometimes did not go smoothly which would then make it hard for us to bill those sessions. However there was always a work around to make it work for us we just sometimes had to be a little more creative.

### AMD, the EMR you love to hate

I love the ability to send out my custom ratings to each patient prior to the appointment, refer to them in the appointment, and have them built into the final note. The telehealth powered by Zoom is very functional. My experienced biller swears by the PM module. By contrast, the eRx is painfully inefficient, and the eRx team shows little interesting is making it efficient. Love/Hate.

The EHR and PM (billing module) are well designed, efficient, and they work. The portal is very functional: patients can make appointments, make payments, see what medications were sent and the pharmacy to which they were sent, make out forms and submit them. Overall, the EHR and PM are powerful, if expensive, but you get what you pay for.

#### Cons

The eRx function is very inefficient. For example, to resend 4 medications, you have to select each one, hit "Resend" which then requires you to send each one individually, or save each one to the main console, select each one again, and then send. To resend 3 CII medications, you have to rebuild one Rx, which then can be duplicated, save to console, then reselect, and then send. AMD uses DrFirst EPCS which requires you to select each scheduled med individually, and then in the EPCS you have to select each one again, individually, enter a password, and then enter a rolled 6 digit code from Symantec VIP (rather than tapping ID.me's column that pops up on your phone. The most recent update actually removed a "Quick Resend" feature so that now, as described above, you have to click each Rx to resend at least twice instead of once and send. The AMD team has made it clear they are not going to restore this feature in response to the heated negative feedback in the "Suggestions"/feedback box. Be sure to have your salesperson walk you through the eRx function before committing to AMD to be sure you know the inefficiency and lack of user response you are getting into.

When I first purchased the program, they lied about its compatibility with Mac iOS. I had to buy an IBM clone laptop to get the software to work. After a year or so, it became compatible with Macs. Then, they loved to update their system during the week, which invariably caused an outage of service for 4 to 24 hours. They are currently better at this. Now, they have made e-prescribing a burden, increasing physician work and decreasing the time the physician has to interact with the patient. They have not been responsive to multiple physicians and NPs asking them to fix this. Indeed, they seem proud that they have made e-prescribing much more difficult and time consuming!!!

It is very customizable. The ability to create your own templates and documents.

Setup is a challenge. Many of our clients are frustrated with AMD and think it stinks simply because they have setup issues with preferences, templates, etc. Once fixed, they love it.

Once you learn how to navigate the system it is fairly straight forward. Integrating billing features into the chart is fairly easy.

#### Cons

Where to start... Not only is AdvancedMD not intuitive, it is plagued with glitches and system failures. The Dashboard and rooming feature rarely works for most of the staff. The e-scribing is terrible. Chart notes are cumbersome and become huge without essential integrations. Auto population of information is elusive and only goes so far (eg. a chart summary does not auto-populate with a patient's BMI from that day. It doesn't matter how good you are at programming and customizing templates, a lot of necessary features are not available (you learn this after spending hours on the phone with customer support.)

Very frustrated from last year. As total system not available for over 24/48 hours or few hours. Help desk doing its job but those folks do not have all the answers or solutions for system failure. Sad part non of the Management or concern department ever contacted or written to explain what had gone wrong. We need explanation for Confidence in the Software / Technology and the team behind it.

Is a very good software but with growth AdvancedMD is losing its focus.

We have spent countless hours over the pasts 2 months trying to tech support for repeated software issues, and brown outs. Despite hours of lost productivity, and revenue, we have maintained a consistent effort to work with your team, and software. Despite this - we are continuing to a see decline in software functionality, and as a result are seeing massive impacts on our day to day operations and revenue. This is unacceptable. The basic premise of our contractual agreement with you, is that we should be receiving at least basic connectivity for the thousands of dollars we pay you. monthly. its been a norm every month either system or some function not available after a patch or release. Speed degradation is common complain. Our Practice, and patients and staff suffer as a result, and we lost lot our reputation with Patients and big financial lose for practice.

If you're looking for a powerful billing platform this may be a good system for you to consider. However, if you're looking for a full PM/EHR system to manage a mental health practice this system is not ready and costs too much for what you get.

- Great billing platform with many ways to keep track of claims and payments - We're able to manage 3 locations with a single system - The system overall has a lot of great capability and most of the functionality we were looking for in a PM/EHR system. However, as noted below there are issues with how some of these capabilities work. - Templates can be customized

#### Cons

- The EHR is not user friendly and leads to many new users creating notes that are not connected to a session as well as multiple redundant notes for a session. - There is no easy way to track missing notes other than going session by session for each chart - The standard reports are generally not very helpful as they provide few ways to customize and filter. The filters that are present are often not logical ways to filter data. What's more frustrating is that there are actually more useful reports that are in the system but they cost additional money to be added to a user's key. For example, a common filter type is to filter a range of providers alphabetically. AdvancedInsight does provide an enhanced reporting capability (for an extra cost) that relies on Internet Explorer and MS Silverlight all of which are old technologies. It is also somewhat difficult and time consuming to learn and still does not provide the ability to customize reports in the standard report capability. For that you have to pay AMD to make the changes or to add the report that already exists to your key. - Printing chart notes requires a time consuming process of creating a Microsoft Word Merge template for each note template and then printing each note one by one. The Word Merge templates are complex to create and manage. - Our sales rep guaranteed us a 95% first pass acceptance rate. After 7 months we are at 89%. There are several reasons for this and the company has provided us a credit but don't be fooled by the promises of their sales team. - Adding new providers can take up to a month. Again we have been promised a 24 hour turn around time and yet sometimes have to wait weeks to get new providers added to our system. Note: we have the ability to add Users but only AMD can add providers.

Advanced MD has helped our Wellness Center achieve it's streamiling and financial goals since it was implemented. When I became the manager here, implementing Advanced MD was one of my first projects and it turned out to be my most crucial. Our practice wouldn't be in the great place we are today without implementing Advanced MD.

TERRIBLE. Just don't even look at this software. There are several other medical record systems out there that work better and are far less expensive. I regret I didn't change over to one of them but it would have been an incredible hassle to do so, especially when you have very limited staff and set up is so time consuming.

That I'm retiring and don't have to deal with it anymore. I selected it becasue I'm visually impaired and for he most part it is easy to see. That's all i like about it. I wish I have chosen another system years ago.

Gouging for every add on. It could cost over $1,000 monthly with any bells and whistles added. Basic service was %730. i actually had a customner service rep hang up on me at 7PM. The have a few good people, and with the Javascript errors and other issues we've had they keep busy.. Long waits on the phone are the rule. They may take days to resolve an issue. No autosave or other means of recovering lost/ acicdentally deleted notes- and it isn't difficult to delete your work- and the customer ttraining is not that helpful. i have to design all my own templates. The sysrem, as I'm writibng this, is down for unknown reasons and of course, they're closed after 7PM central time. wE HAD lots OF PROBLEMS SCANNING IN labs, reports, etc. Every lab interface costs $40 a month. Telemed costs $130 a month. And, the gouges go on and on. i'm fed up and closing my office and they want $3,500 to download my data to disk. I'm a solo practitioner and don't have that much data to download. I'm hiring an IT guy to do it for a tiny fraction of their chartge.

We often feel mislead or duped thinking back on the sales pitch. Everything is 18 clicks- EVERYTHING. Nothing is a one click done, its multiple windows with multiple clicks. Every feature is hidden in a menu in a pop up menu in 5 different screens that after 6 months I still dont know where to find the cool features I was sold on. The absolute worst part is everything is a pop up window. Forget about it on a mac. Nothing is easy with this program. We have wanted to back out and go to a different EHR 100 times since starting the process. We have lost so much money in claims, payroll hours of staff trying to figure out WHY -- WHY WOULD THIS GO HERE? Nothing is easy in this system. I knew something was wrong abut a month ago when money stopped flowing and I just figured it out. Our tax id is incorrect --- HOW COULD THIS HAPPEN? How could this slip through the cracks. Its so hard to be mad because all of the support is so kind and caring and they do their best. Its not their fault this system needs an update or an overhaul. The support staff is the only reason we have stayed so long. They deserve a raise. We pay over $400 a month for this system for 1 provider who prescribes. I feel like its too late now to switch again.I also feel like we are past the "I hate new things" part and this is really the flaws of the system. I came on THIS WEBSITE and it kept saying "easy to use". This is not easy to use in any respect.

I like the customer support (after extremely long hold times) the most. All employees are local, caring and knowledgeable. The implementation staff that we were assigned to were awesome.

All of it. Pop up windows, everything is a pop up. There is no spell check when providers are writing notes. You have to refresh the window to see the changes you made. The system has a DOS feel. Very clunky.

Overall, it worked fairly well for what we needed from it, and was more user-friendly than a couple programs I used in the past (whose names I can't recall). On the other hand, software bugs were common, and were harmful to productivity. Customer support was relatively helpful when needed.

Every employee can use the same program, including both medical and counseling personnel. This integration means everyone can reference notes as needed from both sides. The note templates (text fields, drop-down lists, tabs) can also be changed by administrators as needed and as preferred, so if, say, a counseling assessment is found to likely benefit from new questions, those can be added as new text fields.

#### Cons

In constant daily use, bugs were common. Sometimes AdvancedMD wouldn't play nicely with Chrome, and the cache and cookies had to be reset. Other times, a note that may have been started while online wouldn't be savable if the internet went off then back on. The settings and personalization for users could be difficult to find, understand, or to make certain organizing oe aesthetic changes that would helpful.

Templates are good. there is not much good to talk about. Any thing good is neutralized by amount of problems.

I am a physician that keep loosing my nosed when my medical assistant or front office go in to chart at same time save the chart before me. It also happens when some scans a documents and faxes a prescription on my behalf. I have the RCM contract. EHR is Ok to use.. But they have multiple unscheduled maintenance during night time. Scheduled maintenance is only for 15 minutes and is very occasional. Unscheduled maintenance is very common and last hours. When you call them they make lot of excuses. Billers are inexperienced. Lack knowledge of local insurance and Medicaid requirement. I have complained to them multiple times that they cannot bill medicaid clients for unpaid balances but still they keep mailing them statements. It is more than a year and they continue to bill my Medicaid patients. Very bad on follow up after filing claims. My claims have gone past 95 days filing deadline without any update from advanced. You have to open a case with concerns that there is no claim on file with insurance and it already past filing dead line. Then they will file the claim and claim gets denied. Then we open case again to request them to appeals. They file the appeal and appeal gets denied and you lost your money. They also do not apply payment correctly and you request them to fix the errors , they will not respond. AdvancedMd does not take responsibility for claims being denied for going past deadlines. If you complain they do not respond to your emails.

We were never able to utilize the software. We received inaccurate information during the contracting process; then discovered all the questions we asked ahead of time and had answered, we answered incorrectly.

Avoid this company like the plague. Despite potentially having much higher ratings; our experience with this company has been awful. We asked the right questions; but received the wrong information. This was sadly not identified until after the contract was written. These folks are hands off and provide no assistance. The moment we discovered the numerous issues and reported them (ahead of our scheduled date of implementation), we waited over 30 days for an answer. The long awaited answer was “tough”. “Our finance team decided that since a contract was signed, we will require 90 days of payments to allow you out of a contract”. We never implemented the system, never activated a single employee or provider, and are unable to utilize the software. However, we will be paying for 3 months of service for absolutely no benefit. We only identified 1 single ethical employee in the entire organization. They even debited our account a month ahead of the agreed term. Don’t waste your time. Their are many other SaaS providers available whom are much less expensive, much more helpful, much more ethical, and will aid in the successful operation of your practice/clinic!

Multiple screens available at one time is helpful. More functional PM than Health Fusion, but that's not saying a lot.

Customer Support is TERRIBLE! We were conned into believing that we would be fast tracked and set-up within two weeks. We had a lot of issues with getting Blue Cross Blue Shield set-up, and then e-remittance suddenly stopped after two months. This issues has still not been resolved 5 months later, and ADP keeps trying to send up to BCBS who says everything is fine on their end and that it's ADP's issue. WORSE, it's been 7 months and we are still not getting paid for Medicaid. We have called ADP multiple times, as well as Medicaid, and just today a Medicaid Rep took 5 minutes and figured out that our taxonomy code was listed in the box where our TPI was supposed to be. We called ADP, and they tried to tell us it was our fault despite the fact that we do not enter that info -- they do. They then gave us a run around saying that they only put in standard data, but extra stuff is our responsibility. What?!?! How are we supposed to know that when they don't tell us, and how is putting in a TPI not part of standard date? ADP made it appear that all we had to do was fill out our information, send it to them, and they would handle everything else. We have still yet to hear a response from our direct rep after 2 weeks. I forwarded the emails to customer service 3 days ago, and have yet to hear from them. My office manager called asked to speak with a supervisor, and the customer service rep said, "What are you planning to get out of that?" I don't know. An apology? Most of the reps have been just as rude when we call. You would think the system would flag something when every single Medicaid claim was denied. Or note that we've been calling and emailing since February. I use Practice Fusion for my EHR, and their customer service is much better despite being FREE! I wish they had a PM side, too, so we wouldn't have to deal with ADP, anymore. There's always Kareo, I guess.

### Occupational Therapists DO NOT USE

AFTER I signed a one year contract, my Implementation Trainer revealed she was a specialist in mental health. There was NO specialist in THERAPY. There were NO Occupational Therapy templates I could modify. I was told I could PAY MORE for someone else in another department to create all the templates I was promised and shown. The sales manager talked with me after several weeks and AGREED they could not and did not meet my needs. However, there has been ZERO communication after that. No return calls, no return emails. I continue to be charged monthly for a very expensive service I can not access. The unprofessional operation of such a large company is surprising and unethical. It is a breach if contract-yet I have not been released. The negative impact on my start-up company had been DEVASTATING, yet I can not get a response. I DO NOT RECOMMEND ADVANCED MD unless you are well established and not Occupational Therapy. I don't trust anything they say, as there is no follow up or consistently.

Has everything for most medical practices. Integrated billing.

The software is great for software technicians and highly skilled medical office staff. However, it is NOT good for therapists. I was promised software that would have OT templates I could modify to meet my needs. I was shown examples of this. I was given a sob story of how they would hold my hand through the whole process. Unfortunately, the really was extremely different! There was no software to meet my needs. The creation of forms requires a degree in coding.
