Chronic Watch

ChronicWatch is a mobile-first chronic care management, telemonitoring, and remote patient monitoring software for U.S. care providers and their patients. The platform enables the creation, management and delivery of chronic care plans to patients, in collaboration with home health agencies and patients themselves, who can access the platform at home. ChronicWatch enables the provision of personalized care plans for patients with 2 or more chronic conditions. Patients are able to track their own vitals at home, and through ChronicWatch, share these automatically with their physician for review.
