I missed numerous deadlines and opportunities to gain customers because I couldn't get these forms set up for the beginning of our school year (we're a music school and these were our registration forms). This has been my single worst customer service/support experience ever.

If it has worked, it would have been great that you can process credit cards right there in the form. It's also nice how you can save form sections, and there are many options for question/answer templates.

#### Cons

As soon as I started using Formstack, there was an issue with the payment processor integration where credit card transactions would not go through. I started out using Stripe, and when it didn't work, I contacted support, but none of their solutions fixed it, so I switched to Paypal. I then had the same issues with Paypal. I emailed back and forth with Support for a long time. They offered me the same 4 solutions repeatedly, all of which I tried. I explained to Support that these things weren't helping but it was as if they had amnesia and didn't remember they had already suggested the same things, which didn't work. I sent a message to the Sales team instead to see if they could get me in touch with a different Support professional who knew how to fix this problem (no phone #s on website). Sales said they would escalate the issue. When I didn't hear back for a week, I emailed again asking to cancel my account, and the Sales person said they had emailed support and that "the issue is with your payment processor," which of course I already knew and was the reason for my message in the first place. They continued to insist that the issue was with Paypal. I talked to Paypal support and the problem was not on their end. Many annoying issues with the form creation interface too- like to move questions you have to drag and drop by scrolling through the entire form. And the item/event purchase requires you to give the option of ordering more than one of each item.

Formstack has implemented strict security measures to safeguard sensitive data, whether it is being collected, shared, or stored. Security passwords can be used to encrypt data and guarantee that only individuals with the proper authorization can access particular data. Additionally, workflow approval is in place, so users may only access the form through authorized inputs. Users are able to closely monitor and keep track of every form entry thanks to this.

#### Cons

Better back-end form storing organization structure. The capacity to replicate forms and guarantee that every rule is copied over. High-end plans are the only ones that offer advanced base features like payments, approval, multiple users, single sign-in for forms, and more.

Overall, formstack was a great choice for our organization. We've setup and deployed dozens of forms for both public facing use and internal use.

Formstack's form builder is easy to use and has a lot of integration options. Our form building users range from not-so-tech savvy to power users and both are able to create forms with ease.

#### Cons

Lack of variety with field types. While formstack isn't a "survey" tool, it's a form tool, I think they could implement a lot of field types that survey tools use and make it easier to use a single platform for all form/data collection needs.

#### Reasons for choosing Formstack Forms

While FormAssembly came highly recommended for it's well built Salesforce integration, we found for our needs Formstack was nearly as capable and the pricing per user was much better. With Formstack we get 10 user accounts where I believe a similar price range with Formassembly was for a single user.

At first, our experience with Formstack was promising. However, it clearly became evident that formstack for Salesforce was ill-equipped to handle extensive forms with little formatting needs without passing data to the object.

We like that the product was a native salesforce integration and that it was capable of attaching the response PDF to the account object. Unfortunately, Formstack failed when it came to our formatting needs.

#### Cons

The BioSpine Institute needed a short term patient paperwork solution to attached the response PDF to the Account Object. Unfortunately, we ran into several PDF formatting and rules issues. Our forms required hundreds of rules to hide and show fields based on the patient's input. The rules engine was not user friendly and cumbersome, forcing the user to search for existing rules by clicking through pages. We spent weeks trying to make it work. Our PDFs were 26 pages long when they should have been closer to 8. It was so long because hiding certain pages was impossible. Formstack Forms produced an unacceptable product as it would slow and hinder Clinic operations. Formstack's solution? Formstack suggested using Documents, a feature we had not purchased, to format the PDF the exact way we wanted it. Unfortunately, this would require us to create hundreds of fields in Salesforce and pass the data from the form into Salesforce. We specifically discussed during the sales process that we wanted very little if any patient data in Salesforce. If this were known at the time, we would have looked for another solution. After multiple meetings and communications, we expressed our desire to cancel the contract due to Formstack's inability to meet our needs as promised. However, Formstack denied our request and is currently billing us every month for a product we can't use. If you're looking for a form solution, don't use Formstack for Salesforce.

We need a native form solution that could attache the response PDF to the Account Object. This works, but you have little control over formatting without also purchasing the Documents product. But in order to use the documents product, you must build the fields on the account object to store the data. This was a no go for us and discussed during the sales meeting.

It actually does merge Salesforce data into PDF form fields... eventually, anyway.

#### Cons

It's wildly buggy and inconsistent. Admin portal is often a month behind, so billing display errors create unneeded concerns on an almost monthly basis. And if ever you need tech support you must fight your way through 3 level-one novices who barely comprehend American English before you finally get someone who knows what they're doing. Also, their business practices are generally shady and they honestly shouldn't be trusted. Oh, plus they raised their already expensive fee as if they were worth it - they're not. Nintex/Drawloop is 1000% better at only 50% of the annual cost, we should have switched years ago.

At the time, it seemed less expensive and didn't require an up-front annual payment. But the nonsense limit on allowed template quantity creates a hidden fee that you can't realize until you're more familiar with the platform, and by then it's too late.

#### Pros

Love the incredible ability for logic, detail, customization, calculations, integrations with payment solutions, etc. It's really simple to build a simple form, and with a little additional work for larger or more complex forms, you can intake a huge amount of information with relative ease for both you and the end user. Used this for event registrations for years, and the yearly fee vs paying per transaction made this an affordable solution for us.

#### Cons

Occasionally the modules were tricky to move around when I was working with literally dozens of them. Would have enjoyed the ability to have a button to automatically add a segment of fields and re-add that same segment over and over rather than including them all at once (for example, in a group registration form, having a button that added an additional field for each new group member, rather than a permanently visible set of fields that might not all be needed for a smaller group size). Realizing this is a pretty specific need, we worked around it.

Formstack has features (such as conditions and logic) that allow you to make customizable and efficient forms to reduce survey exhaustion. Google forms is ok for basic forms and great since it's free, but limited in its features. Formstack is the comprehensive software for complex forms.

#### Cons

My one criticism is that it was difficult to view and share all the file submissions from forms. We would collect quite a few file attachments from each form submission and have to download them all and reupload them to a cloud to share with team members.

Great, couldn't be better. You can produce forms very easily with a drag and drop interface. Styling the forms within Formstack is ok; perhaps not as many freedoms as other products but it did everything I needed it to do.

A really simple entry point into building online forms for any website. Simple to use as a standalone product i.e. producing a form for an organisation and hosting direct with Formstack; or embedding so it's on your own URL.

#### Cons

There isn't really anything I didn't like. I simply started working more with Wordpress and found it easier to deploy Gravity Forms given it's native within Wordpress. I believe that Formstack now also offers this.

We use this for new customer registration and have it attached to a payment portal. It's great because the customer can check box all the agreements and use one signature for everything. Families can fill out all their information and Pay in less than 5 minutes. HUGE time saver and we get more business, because people don't have to ever stop and come back to it or flip through pages. If you are a martial arts school and are trying to figure out how to do new student intake and payment without using the really expensive companies like Mind Body or Zen Planner. I highly recommend using along side a merchant system like MXmerchant or a couple of the others Formstack has listed on their website and is compatible with. As soon as I started the trial it was easy to navigate through, and I could pretty quickly figure out what I was looking for. Everything is organized intuitively for a new user. That is probably what made me happiest about it. It also has great options for keeping data protected. That was something we struggles with a lot looking at other form generators. I can also generate a ton of different forms at no extra cost. I even use this if parents are signing for our annual potluck.

#### Cons

I don't get a lot of control manipulating the text for each field or heading. My biggest struggle is making the forms easy fora customers eyes to follow the page and be easy to fill out. When all the text is stuck the same size, the question and list of checkbox answers, it is kind of overwhelming, and customers in a hurry get lost and have to go back and re-read.

The time and money saved driving to get applications signed, the mail and waiting forever for the client to sign and mail back. Formstack is quick and easy and saves so much time and energy!

How easy it is to use for the user and for the client that we send it to . Its only a couple clicks away and takes seconds. Also, when I print to Formstack, a little box comes up and takes me right there and the document is ready to go.

This recently starting happening, after I print to Formstack and the little box comes up to print I use to click on it and it would take me right to the document. Now it is taking me to my profile/account and i cant get out of it. I now have to close out of it and reopen the website again. This wastes so much time.

Horrible. If I could use the account, maybe I'd have a better experience. Even trying to get access was a challenge. If your company's grown over the past 2 years, perhaps it's time to hire more CSRs.

Lately, nothing. The forms are fine to create, drag and drop's fine, etc. But then there's limits on how many responses you can receive. If using this as an ongoing product, eventually you're going to reach that limit. How's one to know the quantity of responses they'll need for a recurring form?

#### Cons

There's no contact. I can't seem to get a single human to reply to multiple support and billing issues to the point that my account's now suspended. ZERO human reply. Automated replies with no actual help. I don't recommend them because if you can't get support when you need it, what's the point?

Overall I recommend this product for small businesses that need a tool to help them collect customer information and/or process payments. It is helpful that it has logic that allows you to better customize to customer needs. For example, we use it to collect cancellations and can upsell / cross sell to people based on the reason the cancel.

I like that it's easy to use and can be self taught. It is also helpful in that you can use it to process credit card payments and embed the form on your website if you don't have that software already built into your site. I've also found the "live" form that shows up right away to be a useful tool to determine if the form is built correctly and useful.

#### Cons

The formatting can be tricky and if you are building out logic it can be confusing. I've found myself making mistakes occasionally because of the logic and it not being clear what I'm about to do before I do it.

Great options for communicating with your survey users - you can set it so they are automatically emailed a response and also customize the landing page they are taken to after completing the survey.

There's a lot you can do with forms using this program. You can customize the URL of the form and easily embed it into other sites.

Sometimes when I duplicated forms, I noticed that the logic would get lost. This was frustrating as I would have to go through the form again to add the missing parts of the logic back. We also always had problems with people who saved their surveys being able to access them again.

I hated that you couldn't get ahold of any one. If we needed a form revamped, only they could do it and connecting with them was impossible. It became an impossibility of performance and we had to leave.

I think that they potentially could have given us usable forms, but the process was too cumbersome and the service was poor.

#### Cons

They wouldn't work with us and we opted to cancel, but they refused to release us from what they thought were contractual obligations. If you don't deliver what we contract for, then you better believe that we are not going to pay. They are unyielding. We paid over $2500 for nothing.

This web based form builder is more than that. It helps you create and collect information from forms that you customize. It also has various integrations that you can use to advance your forms the way it fits your organization. Super.

I did not have any big issues with using the web based form builder. Its simple and very easy to use. Literally drag and drop.

#### Pros

Creating complex forms was easy for me and I’m not a programmer. Lots of options for how to present and collect responses. Good integration with Google sheets for using the data after collection.

#### Cons

This is specific, but bummed it doesn’t integrate easily with NetSuite, which we just started using this year.

We are accepting payments, gifts, and facilitating registration/ticketing with Formstack, and although it needs some work on the presentation aspect, it is a great and economical tool for nonprofits.

#### Pros

The simplicity of Formstack is its greatest benefit. It is extremely simple to set up, use (on the client-facing side), and receipt.

#### Cons

The additional functionality and presentation are absent with Formstack. We still use Formstack (and will probably continue to use it) because it doesn't have many problems, but from the client-facing side, the forms can look like they are from the early 2000's.
