Momentum Healthware’s Care Management system offers an electronic health record (EHR) solution with multiple applications for long-term care providers. This Microsoft-based software serves small to midsized facilities and home care agencies in the United States and Canada.

Available modules for clinical needs include digital physician order forms, progress notes and Minimum Data Set (MDS) assessments. Resident charts can be stored and edited electronically, so they're accessible from multiple locations. A touchscreen interface allows users to enter patients’ activities of daily living (ADL) information.

Momentum Healthware enables caregivers to report medical incidents, such as infections or medication errors. Afterward, email alerts can be sent to specific staff members to track and assess a specific incident’s follow-up care strategy.

Financial processes can also be automated with this software. Administrators can assign recurring charges to a resident account during each billing cycle, and any number of payers can be entered into the system.
