Humhealth is a group of programs that supports RPM, CCM, AWV, BHI, PCM, TCM, and RTM.

Humhealth provides best-in-class, easy-to-use chronic care management services. Humhealth platform ensures the successful completion of all activities ranging from identifying and enrolling the right patients, all the way to being reimbursed after the end of a care cycle.

For clinicians, it allows them to add other care team members, engage in two-way communication, assess patients remotely, create and personalize care plans, monitor the patient’s condition, and not worry about tracking time for all relevant chronic care management services provided.

For patients, it ensures that they stay connected with the rest of their care team, receive instructions and educational materials remotely, and track their lifestyle, which includes adhering to the right diet, nutrition and medications.

Any reimbursable chronic care management services or activities are automatically captured by the timer that is built into our platform. This includes the time spent by clinicians and their team defining a chronic care management plan and conducting non-face-to-face follow-up calls every month.

Great interface for healthcare patients

I recently had to go to the doctor for some healthcare issues, and the office used HumHealth. Everything about the software (from my end, as a consumer) was easy and simple.

The most important focus in our industry is patient management. This is where Humhealth excels. The platform is designed for ease of use for the provider, patient, and support staff. If you are a service provider and want to establiosh an RPM/CCM program in place for your patients, you will recevie detailed and timely support from the team at Humhealth. I feel like [sensitive content hidden] , the [sensitive content hidden] of the Humhealth, has brought together a team of professionals that treat their job like a mission. [sensitive content hidden] has been my point person at Humhealth. He has been indispensable in the creation of my company and the service we provide to our clients. He scheduled several meetings to walk us through the website and how to create various levels of access based on clinical need, support staff, and patient access.Additionally, there are many technical unknowns for our startup that [sensitive content hidden] help us navigate efficiently. I'm not sure where we would be in our progress without his support and advice. I truly want to share a more detailed experience we have with Humhealth for any providers or service companies looking to incorporate RPM and CCM into their practice. The team truly feels like a partner in our business. We are not just their customer. Lastly, there is a reason I did not mention anything about pricing. The company's pricing is one of the most competitive in the industry, but more importantly, the service, support, and advice is much more valuable.

Cons:

No company, nor any individual is perfect, but I truly don't have any cons to share at this point in my experience with Humhealth. I will give updates later as our journey with Humhealth continues.

Cons:

Small entry boxes and repetitive date changes to make updates to previous entries are time-consuming and tedious. Also, the lack of a drop down to start/stop service timers under the correct service name should be implemented.

RELIABLE AND EFFICENT -PATIENT CARE PLATFORM

I’ve been using this for the past two years for CCM, and I’m very satisfied. It’s easy to navigate, helps track the CCM timer, and the team provides great training and quick support whenever issues come up.

The Hum Health platform is easy to navigate and user-friendly for staff. Accurate CCM timer tracking, which helps with compliance and billing.

Cons:

It would be even better with more integration options (EHRs,( PRACTICE FUSION Integration, billing systems, etc.).

What I like most about humhealth is that it has more than one use. It has annual wellness visit and chronic care management and can be used for multiple things.

Cons:

What I like least about humhealth is that it does not have many options to customize. Most times I have to use the drop downs it provides with no room to edit.

Platform friendly user and great patient management. Updated with all remote services available. Great customer service. And patient's engagement is easy.

Cons:

No AI integration yet. This should be implemented asap due to circumstances. Also once month's close it is not always easy to make modification on month's prior.

Ability to create Program Templates for ICD structure. Ability to toggle between Programs. Ability/ Ease of clearing RPM Alerts.

Cons:

Transferring Patients to a new Clinician is time consuming and requires a x2 step process to complete the task; reducing this to x1 step would be beneficial. The RPM Program counts 99454 Transmission Days per Calendar Month only. The Administrative Dashboard is well organized and easy to maneuver through while the Clinician Dashboard is busy and overwhelming w/ information not utilized.

The personal attention received upon initiation of this Program. The ability to adapt to our company needs and the price point.

The help you need for chronic patient management

I tried out this app while shadowing a doctor and I must say even not having completed my medical eductaion, the software seemed very convenient and relatively easy to use. The doctor loved that it made billing issues with Medicare patients much easier. The software also made it possible for patient information to be updated even outside of doctor's appointments, which is crucial for chronic illnesses.

Cons:

There seemed to be a lot of parts to fill out that did not apply to certain situations/patients, so if the software would let physicians customize it more, that would make it even better than it already is!
